    Polymorphic ventricular arrhythmia triggered by temporary epicardial right ventricular stimulation after cardiac surgery

    Temporary epicardial pacing is commonly used for rhythmic support in the acute postoperative period after cardiac surgery. Clinical studies suggest that pacing in the left ventricular epicardium during resynchronization therapy may be proarrhythmic in a minority of patients. The epicardial pacing increases transmural heterogeneity of repolarization. This could prolong the QT and JT intervals on the electrocardiogram (ECG), with subsequent proarrhythmic effects.     The flavor-changing neutral-current (FCNC) decay Λþ c → pμþμ− (inclusion of the charge-conjugate processes is implied throughout) is expected to be heavily suppressed in the Standard Model (SM) by the Glashow-IliopoulosMaiani mechanism [1]. The branching fractions for shortdistance c → ulþl− contributions to the transition are expected to be of Oð10−9Þ in the SM but can be enhanced by effects beyond the SM. However, long-distance contributions proceeding via a tree-level amplitude, with an intermediate meson resonance decaying into a dimuon pair [2,3], can increase the branching fraction up to Oð10−6Þ [4]. The short-distance and hadronic contributions can be separated by splitting the data set into relevant regions of dimuon mass. The Λþ c → pμþμ− decay has been previously searched for by the BABAR Collaboration [5], yielding 11.1 5.0 2.5 events and an upper limit on the branching fraction of 4.4 × 10−5 at 90% C.L. Similar FCNC transitions for the b-quark system (b → slþl−) exhibit a pattern of consistent deviations from the current SM predictions both in branching fractions [6] and angular observables [7], with the combined significance reaching 4 to 5 standard deviations [8,9]. Processes involving c → ulþl− transitions are far less explored at both the experimental and theoretical levels, which makes such measurements desirable. Similar analyses of the D system have reported evidence for the longdistance contribution [10]; however, the short-distance contributions have not been established [11]

    El propósito del presente estudio es determinar si una mecánica de tiro alternativa basada en la lateralidad óculo-manual del sujeto podría incrementar significativamente el porcentaje de acierto en el tiro a canasta en jugadores homogéneos – aquellos cuya lateralidad ocular y manual están en el mismo hemicuerpo lateral –. Para esto, 34 jugadores de baloncesto (24chicos y 10 chicas) en categoría infantil, inscritos todos ellos en categoría Preferente masculino y femenino Federado, realizaron un total de 4600 lanzamientos a canasta desde cinco posiciones diferentes de corta y media distancia - 2300 con cada una de las dos mecánicas propuestas (mecánica tradicional y tiro con rotación de 45º) -. Tras el análisis de datos se hallaron porcentajes de acierto significativamente mayores desde el tiro libre (t(23) =2,917; p = 0,008), los lanzamientos desde el lado izquierdo de la zona (t(23) = 3,621; p = 0,001) y el global de los lanzamientos a canasta (t(23) = 3,980; p = 0,001) con la mecánica de tiro teóricamente idónea. Los resultados obtenidos sustentan que una mecánica de tiro alternativa basada en la lateralidad óculo manual del jugador podría ayudar a los jugadores de baloncesto homogéneos a obtener porcentajes de acierto significativamente superiores

    Subcutaneous metastasis from a visceral malignancy is rare with an incidence of 5.3%. Skin involvement as the presenting sign of a silent internal malignancy is an even rarer event occurring in approximately 0.8%. We report a case of a patient who presented to her dermatologist complaining of rapidly developing subcutaneous nodules which subsequently proved to be metastatic colon cancer, and we provide a review of the literature

    The Hierarchical Structure and Dynamics of Voids

    Contrary to the common view voids have very complex internal structure and dynamics. Here we show how the hierarchy of structures in the density field inside voids is reflected by a similar hierarchy of structures in the velocity field. Voids defined by dense filaments and clusters can de described as simple expanding domains with coherent flows everywhere except at their boundaries. At scales smaller that the void radius the velocity field breaks into expanding sub-domains corresponding to sub- voids. These sub-domains break into even smaller sub-sub domains at smaller scales resulting in a nesting hierarchy of locally expanding domains. The ratio between the magnitude of the velocity field responsible for the expansion of the void and the velocity field defining the sub voids is approximately one order of magnitude. The small-scale components of the velocity field play a minor role in the shaping of the voids but they define the local dynamics directly affecting the processes of galaxy formation and evolution. The super-Hubble expansion inside voids makes them cosmic magnifiers by stretching their internal primordial density fluctuations allowing us to probe the small scales in the primordial density field. Voids also act like time machines by "freezing" the development of the medium-scale density fluctuations responsible for the formation of the tenuous web of structures seen connecting proto galaxies in computer simulations. As a result of this freezing haloes in voids can remain "connected" to this tenuous web until the present time. This may have an important effect in the formation and evolution of galaxies in voids by providing an efficient gas accretion mechanism via coherent low-velocity streams that can keep a steady inflow of matter for extended periods of time.Comment: High-res version are related media here: http://skysrv.pha.jhu.edu/~miguel/Papers/Hierarchy_voids/index.htm

    Acquired potential N-glycosylation sites within the tumor-specific immunoglobulin heavy chains of B-cell malignancies

    Background and Objectives. Among B-cell malignancies, follicular lymphomas (FL) more frequently show acquired, potential N-glycosylation sites (AGS) within tumor-specific immunoglobulin. The aim of this study was to extend this observation and to evaluate the pattern of presentation of AGS within five different forms of B-cell lymphoma. Design and Methods. We sequenced the tumor-specific immunoglobulin heavy chain variable region fragment, including complementarity-determining regions 2 and 3, of forty-seven consecutive patients with a B-cell malignancy enrolled in idiotype vaccine clinical trials. This sequencing approach is known to allow the identification of most AGS. We then statistically analyzed differences in presentation pattern, in terms of tumor histology, immunoglobulin isotype, AGS location and amino acid composition. Results. All twenty-four FL cases presented with at least one AGS, whereas the vast majority of four B-cell lymphoma types other than FL did not. The non- FL group of tumors included four cases of Burkitt’s lymphoma, six of diffuse large cell lymphoma, seven mantle cell lymphomas and six small lymphocytic lymphomas. Most IgM-bearing follicular lymphoma cases featured their AGS within complementarity-determining region 2, as opposed to those bearing an IgG, which mostly displayed the AGS within complementarity- determining region 3. The vast majority of AGS located within either complementarity- determining region ended with a serine residue, whereas those located within framework regions mostly featured threonine as the last amino acid residue. Interpretation and Conclusions. In our series, all cases of FL had AGS within their tumor-specific immunoglobulin heavy chain variable regions. In contrast, most B-cell malignancies other than FL did not. Further studies are warranted in order to establish the possible meaning of these findings in terms of disease pathogenesis, their diagnostic value in doubtful cases and their potential implications for immunotherapy
